The obverse side of the medal features colour figure of Rákosníček and his name in the lower part. A composition comprising characters from Zdeněk Smetana’s fairy tales is depicted on the reverse side as well as the text “Pohádky Zdeňka Smetany“ (Zdeněk Smetana’s fairy tales).
The design was transformed into three-dimensional form by Asamat Baltaev, the graduate of the Medallist School in Jablonec.
